Soteria Care Pty Ltd (ABN 27 694 612 121) provides physiotherapy services in Sydney. We are bound by the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records and Information Privacy Act 2002 (NSW). This policy explains how we handle your personal and health information.
What we collect
We collect information needed to provide and coordinate your care, including your name, contact details, date of birth, health and medical history, assessment and treatment notes, funding details (such as NDIS plan, health fund, Medicare, insurer or claim information), and the contact details of your GP, support coordinator, plan manager or case manager.
When you send an enquiry through our website we collect the details you enter in the form: your name, phone number, email address, suburb, funding type, preferred session location and message.
How we collect it
We collect information directly from you or your authorised representative wherever possible. With your consent, we may also receive it from referrers such as GPs, support coordinators, plan managers, insurers and other health professionals.
How we use it
- To respond to your enquiry and book appointments
- To assess, treat and manage your care
- To write reports and communicate with your care team, with your consent
- To process payments and claims with the NDIS, plan managers, health funds, Medicare, insurers or other funders
- To meet our legal, professional and record-keeping obligations
We do not sell your information or use it for marketing without your consent.
Who we share it with
We only share your information with people involved in your care or funding, such as your GP, support coordinator, plan manager, insurer or other health professionals, and only with your consent or where the law requires or allows it. We use secure service providers to store records, manage email and process website enquiries. Some of these providers may store data outside Australia; we take reasonable steps to make sure they protect it to the standard required by Australian law.
How we store and protect it
Records are stored electronically on secure, password-protected systems with access limited to authorised staff. Health records are kept for at least seven years after your last visit, or until a child turns 25, as required by NSW law, and are then securely destroyed.
Accessing and correcting your information
You can ask to see or correct the information we hold about you by contacting us. We will respond within 30 days. In rare cases the law allows us to refuse access; if so, we will explain why.
Our website
Our website does not use advertising or tracking cookies. Enquiries submitted through the website are emailed to us and stored securely in our practice records.
Complaints
If you have a concern about how we have handled your information, please contact us first so we can try to resolve it. If you are not satisfied, you can contact the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992) or the NSW Information and Privacy Commission (ipc.nsw.gov.au, 1800 472 679). NDIS participants can also contact the NDIS Quality and Safeguards Commission (1800 035 544).
